L.A. Care Health Plan Commits $4.7 Million in New Round of Grants to Transform Medi-Cal

These Grants Bring Total Commitment to More Than $20 Million

LOS ANGELES – L.A. Care Health Plan is proud to announce that it has committed $4,732,298 in Incentive Payment Program (IPP) funding to 33 providers and community-based organizations (CBOs), offering Enhanced Care Management and Community Supports as part of California Advancing and Innovating Medi-Cal (CalAIM). This is the health plan’s third round of IPP grants, bringing the total commitment since 2022 to $20,599,835. The funding supports technology upgrades, as well as the recruitment, hiring and training of new staff to help providers deliver the CalAIM services.

“The incentive payments will ensure that these providers and CBOs have the support needed to help CalAIM succeed in offering more equitable and coordinated care for our members and others in Los Angeles County,” said John Baackes, L.A. Care CEO. “We recognize that health care is more than a plastic card in a wallet. This ambitious initiative addresses the critical social needs that are found outside a doctor’s office and helps to improve the wellbeing of our members.”

Twenty-two of the new grantees provide Enhanced Care Management, which addresses the clinical and non-clinical needs of the highest-need members through intensive coordination of health and health-related services. Angeles Del Sol ADHC is one of these grantees.

Chinatown Service Center is among 15 grantees who will provide Community Supports, which include help with housing, recuperative care, medically tailored meals, and other services for L.A. Care members.

“At Chinatown Service Center, we are extremely grateful to be included in this latest IPP Funding opportunity, which enables us to expand our CalAIM services in Chinatown and the San Gabriel Valley,” said Gene Hui, Patient Experience Manager at Chinatown Service Center. “This funding allows us to build up the necessary infrastructure for our Community Supports services, beginning with Asthma Remediation, Housing Tenancy and Sustainability, and Housing Navigation. These services, supported by our Case Managers and Community Health Workers, will help mitigate and address the social determinants of health affecting our clients and patients.”

Four of the grantees in this round are providing both Enhanced Care Management and Community Supports.

Last year, in its second round of IPP grants, L.A. Care committed $2,343,518 to 18 providers. In 2022, in the first round of IPP grants, the health plan committed $13,524,019 to 61 providers. L.A. Care received this funding from the California Department of Health Services (DHCS) to fulfill CalAIM requirements.
