L.A. Care’s Care Management Program is primarily telephonic and designed to help members understand their health needs and treatment plan to improve the quality of their health. Our team consists of nurses, social workers, community health workers, and care coordinators who support members by collaborating with member’s primary care provider, specialists, family members and any necessary supportive and community services.
The overall goal of the Care Management program is to improve member health outcomes by providing comprehensive care coordination and empowering members to become independent with managing their health by:
- Connecting members to integrating services within L.A. Care Health Plan and Preferred Provider Groups (PPGs)
- Decreasing avoidable utilization and duplication of services
- Providing education on how to navigate their health care
- Identifying social drivers of health and facilitating access to community resources

Eligibility
All L.A. Care members are eligible to participate in the Care Management program if they meet specific eligibility criteria.

Transitional Care Services
Medi-Cal members who experience a hospital or facility admission can receive coordination and support to ensure all needed resources and services are received at home.
