Scheduling Follow-up Appointments After Emergency Department Visits

September and October 2024

mom and daughterMany members using Emergency Departments (EDs) for mental illness or substance use issues are not connected to ongoing behavioral care.

Providers can address physical needs and initiate behavioral health conversations after ED visits occur.

Reaching out and scheduling follow-up appointments with your patients helps ensure they can be connected to ongoing treatment and hopefully reduce the number of ED visits.

Ensuring these appointments happen within seven to 30 days of the ED visit also helps support with NCQA Quality Measures, such as FUM and FUH (Follow-Up After ED Visit for Mental Illness and Substance Use).

During your visit, if a patient requires additional behavioral health support, you or the patient can contact Carelon Behavioral Health at 877.344.2858 (TTY 1.800.735.2929) to be connected to a provider today!
